At Bethany Lutheran Home, everything revolves around providing the best possible care for our residents. To achieve that, we create a care plan with the resident, their family and the Bethany care planning team. The Bethany care plan team consists of nurses, a social worker, a dietary manager, the activities coordinator and the therapy director. They review the doctor's orders for each resident, nursing care, family and social strengths, diet and fluid needs, daily activities and interests. Family members contribute helpful information on the interests, history and concerns of the resident. The resident is also encouraged to speak about his or her goals for their stay at Bethany, including choices for their daily routine.

The care plan team meets at least every 90 days to review each resident's care plan, make changes and gather more information. We find this to be a great way to ensure that we are staying on top of our residents and their changing needs. The whole team effort is focused on the resident. Family members are notified in advance of care plan team meetings and they are encouraged to attend. If a family member lives out of the area, we can arrange phone conference calls as well so everyone can stay up-to-date.
